Local Services Ads for orthodontists: no lead credits, parent calls and cost per treatment start
Local Services Ads can bring parents and adults calling to book a consult, but orthodontists sit in health care: no lead credits, no booking leads and no call recordings. It pays only when services and ad hours keep out existing patients and price shoppers, and each lead is matched to a treatment start in Dolphin, Cloud 9 or Edge Cloud.

Symptoms
What LSA problems look like for orthodontists
- 01
LSA charged us for a mom whose son had a poking wire, and there is no button to get the money back.
What it costs
Every patient-of-record call that reaches you through LSA is permanent spend. With no credit path, the only levers are which services are switched on, which hours the ad runs, and making your office number easier for families in treatment to find than the listing.
What’s happening
Google screens orthodontists as health care, and lead credits are not available for health care verticals. The automatic crediting that plumbers and lawyers get for low-quality leads does not apply to you. An existing patient's parent who searches your practice name and taps the LSA call button is a charged lead, full stop.
- 02
The front desk says LSA calls are mostly 'how much are braces' and 'do you take our insurance', and then the parent hangs up.
What it costs
You cannot tell a lead problem from a phone problem. Practices cut a channel that might be working, or keep paying for one that is not, depending on who tells the story at the team meeting.
What’s happening
A price or insurance question is a contact with your business, so it is a valid lead. Google also does not offer call recordings for health care verticals in the US, so you cannot listen back to hear whether the caller was a shopper or whether the front desk lost a family that was ready to book.
- 03
LSA calls get missed every afternoon, when the chairs are full of kids out of school and the front desk is checking in patients.
What it costs
You pay for calls nobody answered, the parent books the next practice on the list, and the missed calls weigh on your ranking too.
What’s happening
A missed call during your business hours is charged once the caller stays on the line past 20 seconds, and a voicemail greeting counts as a connected call. The after-school block is when an orthodontic front desk is least able to pick up, and the listing keeps running straight through it.
- 04
LSA looks cheap per lead, but nobody can say how many treatment starts it produced.
What it costs
LSA gets judged on lead count. A channel that looks cheap per call may be expensive per start, or the reverse, and the decision gets made without knowing which.
What’s happening
A consult call is weeks or months away from a signed treatment contract. LSA leads have no click ID, the caller is usually a parent while the chart belongs to the child, and the referral source field in the practice system says 'Google' for Search, LSA and the Business Profile alike.
The goal
What a fixed LSA account looks like for orthodontists
- Every LSA lead matched to a new-patient record by the parent's or patient's phone number, with an outcome.
- Cost per consult kept and per treatment start from LSA, braces and aligners separately, next to Search.
- Existing-patient calls through LSA counted every month, so you can see whether putting the office number on reminders worked.
- LSA ad hours inside staffed hours, with lunch closures and satellite days reflected.
- No patient or treatment detail sent to any ad platform; the reconciliation stays inside the practice.
Eligibility
What Google checks before orthodontists can run LSA
- 1A provider background check and a practice background check.
- 2State-level licenses for the practice and for each provider, where local law requires them. Each provider clears their own license check, so a practice with several orthodontists is only as fast as its slowest file.
- 3General liability insurance for the practice and professional liability or malpractice insurance for providers, which Google asks of selected businesses.
- 4National Provider Identifier verification, a business registration check and a business representative check, also for selected businesses. Health care verticals cannot run pre-badge ads, so nothing serves until screening is finished.

Job types
LSA job types for orthodontists: what to switch on, what quietly costs money
Switch on
Braces consults for children and teens
The core of most practices' new-patient flow. The call comes from a parent who wants a nearby office and a time that fits around school, so keep it on and make sure the consult can be booked on the first call.
Clear aligner and Invisalign consults for adults
Adults research aligners on their own and call when they have decided to look seriously. Keep it on if you offer aligners, and report these as their own line, because aligner callers compare prices differently from parents of braces patients.
Early orthodontic evaluations
Parents whose family dentist suggested having a young child checked. Worth having on if you see children early, since that first check is where the relationship with the whole family starts.
Adult braces
A smaller group than aligner shoppers, and often past the 'should I' question; they want the right office. Keep it on if adult cases suit your schedule and chairs.
Switch off, or check first
Retainer replacement, if your retainer calls are mostly your own patients
Retainer calls come from your own finished patients or from people treated elsewhere who want a replacement. Neither is a new case, and in health care neither can be credited.
Emergency repairs such as broken brackets and poking wires
These callers are existing patients by definition. Put a direct office and after-hours number in every reminder text and on the aligner case instructions, so they never need to search for you.
Jaw surgery or TMJ treatment, unless you coordinate it in house
Callers want a surgeon or a TMJ specialist and will be referred on, and the lead is charged anyway.
Bad leads
What a bad LSA lead looks like for orthodontists
An existing patient's parent calling about a broken bracket or to move an appointment.
No credit is available. Print the office number on every reminder text and in the patient portal, and keep LSA ad hours inside staffed hours so after-hours repairs reach your on-call line rather than the listing.
A parent who only wants the price of braces and whether you take their plan.
A valid lead with a low booking rate. Train the front desk to answer with the consult offer and whatever fee range you are comfortable quoting, then record whether a consult was booked.
An adult comparing your fee with mail-order aligners.
Record the reason when it does not book. If aligner callers mostly end here, that service may be pulling price shoppers that a Search campaign with its own landing page would qualify better.
A dental lab, a supply rep or a marketing company calling the listing.
Charged, and not creditable in health care. Log it so the share of vendor calls stays visible; if it climbs, tighten the services you show, because no dispute will fix it.
A family referred by a general dentist who searched your practice name.
Valid, charged, and a patient who was already coming. Ask how they heard about you and record the referring office even when the call came through LSA, so the referral is credited to the dentist relationship.
Ranking
What moves your LSA position as one of the orthodontists in the auction
- 01
Reviews
Rating and number of reviews count toward profile quality. Debond day, when the braces come off and the family takes photos, is the natural moment to ask. Keep review replies free of any treatment detail.
- 02
Answering during the after-school rush
Missed calls can hurt ranking. A dedicated new-patient line, overflow to a second desk, or an answering service that can book consults protects the listing in the hours the front desk is busiest.
- 03
Ad hours that match staffed hours
Unanswered calls are charged during your ad schedule and business hours. If the office closes for lunch, or a satellite office opens only some days, set the schedule to match instead of leaving the listing live.
- 04
Photos
High-quality images count toward profile quality. Office, team and equipment photos are safe to post; smile photos need the patient's written permission, and a minor's need a parent's.
- 05
Bid and budget
Google recommends Maximize Leads. Parents often look into braces after work, so a budget spent by early afternoon hands the evening to other practices, but evening calls are only worth buying if someone answers them.

How we work
Getting LSA leads into Dolphin Management, Cloud 9 Ortho or Edge Cloud (Ortho2)
- 1
What to record
A referral source of Google LSA on the new-patient record, with the responsible party's phone number, since the caller is usually the parent and the chart is the child's.
- 2
How to match
Match each charged LSA lead to a new-patient record by the responsible party's phone number and the call time, using the LSA lead list or the Google Ads API, then follow it through consult booked, consult kept and treatment started. Dolphin Management often runs on a practice server, so the matching usually works from an export or a sync partner rather than live.
- 3
Lead feedback
There are no credits to claim in health care, so logging is for your own decisions: existing patient, price-only, insurance-only, vendor, dentist referral, consult booked, started. That list tells you which services and hours to change.
- 4
Monthly check
Monthly for the lead mix and quarterly for starts: LSA leads from earlier months matched to treatment starts and contract value, braces and aligners on separate rows, next to Search. None of it leaves the practice. LSA has no conversion import, and nothing about a patient's treatment should go to an ad platform.
No accuracy guarantee here: we report the share of LSA leads for orthodontists matched to new patients, and the share we could not match.
Channel split
LSA, Search ads and the Business Profile for orthodontists
For an orthodontist the largest free source of new patients is often referring general dentists, and that is where LSA quietly costs money: a family told to 'go see Dr. Lee' searches the practice name, and the LSA call button can sit above your website. Search ads give you control the listing does not: a landing page for adult aligners, another for early evaluations, and treatment starts imported against the original click with neutral conversion names and no diagnosis. Health is a sensitive category under Google's personalized ads policy, so remarketing lists and Customer Match are off the table for these ads either way. LSA's job is the parent who wants a nearby office and a consult time now.
- Local Services Ads
- Consult calls from parents and adults in your core area, judged on treatment starts per lead, with existing-patient calls kept to a minimum.
- Search ads
- Aligner and adult treatment searches with their own pages, plus the hours LSA does not cover, with starts imported against the click and nothing clinical in the conversion.
- Business Profile
- Reviews and office photos every parent compares. Tag its website and appointment links so map consults are not counted as direct.
- Referring dentists and aligner locators
- Tracked by source in the practice system and kept out of paid reporting. Invisalign Doctor Locator requests are captured on the vendor's site, so they can be labeled but never imported as clicks.
Google Business Profile
The Business Profile side for orthodontists
- Primary category
- Orthodontist
- Suspension risk
- Moderate, mostly from satellite offices and departed providers. Many orthodontists run satellite offices that open only some days of the week. Google's guidelines exclude locations that are not staffed during their stated hours, so a satellite profile showing full-week hours, or an associate's practitioner profile left behind after they leave, invites edits and suspension. Names padded with 'Invisalign' or 'Braces Near Me' run against the naming rules as well.
- Address
- Patients come to the office, so show the address. Each staffed location can have its own profile with its real days and hours; a room borrowed once a month should not.
Honest fit check
When LSA is not worth it for orthodontists
- Your next new-patient consult is weeks out. A parent calling from LSA wants a time this month and will simply call the next office.
- Most of your starts come from referring dentists and the schedule is full. LSA would mainly charge you for families who were already sent your way.
- Nobody can answer during the after-school block and there is no overflow. With no credits in health care, every missed call that holds on is a sunk cost.
FAQ
Questions orthodontists ask about Local Services Ads
Can orthodontists get credits for bad LSA leads?
No. Google does not offer lead credits for health care verticals, and its screening page lists Orthodontist under health care. Prevention is the only lever: the services you show, the hours the ad runs, and making sure existing patients have your direct number.
Can parents book a consult directly through LSA?
Not in health care. Google says booking leads are not available for health care verticals, so orthodontic LSA leads arrive as calls and messages. Your online scheduler belongs on the website and the Business Profile, tagged by source.
Can we listen to our LSA call recordings?
Not as a US health care practice; Google does not offer recordings for those verticals. Have the front desk log an outcome for every LSA call, and use your own phone system, with a consent notice your counsel approves, if you want recordings.
Is Local Services Ads worth it for an orthodontic practice?
It is worth it when matched treatment starts, after taking out existing patients and dentist referrals, come in at a cost per start you would pay again. Cost per lead says little when a consult can take months to become a contract.
What can we check ourselves this week?
Export last month's LSA leads and have the front desk look up each number. Count existing patients, price-only or insurance-only calls, referred families and consults booked. Then compare your LSA ad hours with the hours someone is actually free to answer.
Does any patient data go to Google?
Not from the LSA reconciliation. LSA has no conversion import, so the match to starts stays in your practice system and your own report. For Search ads, imported starts carry neutral names and contract value, never a diagnosis or treatment detail.
