Local Services Ads for dentists: no lead credits, patient-of-record calls and cost per new patient

Local Services Ads can fill new-patient exam slots for a dental practice, but dentists are screened as health care: no lead credits, no booking leads and no call recordings. It pays when services, hours and the bio keep out recall and insurance calls, and each lead is matched to a kept first visit in Dentrix, Open Dental or Eaglesoft.

Symptoms

What LSA problems look like for dentists

  1. 01

LSA charged you for a patient of record who called to move her cleaning, and there is no button to get the money back.

    What it costs

Every recall, billing or reschedule call that reaches you through LSA is permanent spend. With no credit path, the only levers are which services are switched on, which hours the listing runs, and how easy you make the office number to find for people already on your schedule.

    What’s happening

Google screens dentists as a health care vertical, and lead credits are not available for health care verticals. The automatic crediting that plumbers and restoration companies get for low-quality leads does not reach a dental office. A hygiene patient who searches your practice name and taps the call button on the LSA listing is a charged lead, full stop.

  2. 02

The LSA calls the front desk remembers start with 'Do you take my insurance?' and end with a hang-up.

    What it costs

You cannot tell a lead problem from a phone problem. Practices drop a channel that might be working, or keep paying for one that is not, depending on who tells the story at the morning huddle.

    What’s happening

An insurance or Medicaid question is a contact with your business, so it is a valid lead. Google does not offer call recordings for health care verticals in the US either, so you cannot listen back to hear whether the caller was a plan shopper or whether the front desk lost someone who was ready to book.

  3. 03

LSA keeps ringing over the lunch hour and on the afternoon the office is closed, and nobody picks up.

    What it costs

You pay for calls nobody answered, the caller books with the next practice on the page, and the missed calls weigh on your responsiveness ranking.

    What’s happening

A missed call during your business hours is charged once the caller stays on the line past 20 seconds, and a voicemail greeting counts as a connected call. If the hours on your listing say open while the front desk is at lunch or the doctor works a short day, the listing keeps selling calls into an empty office.

  4. 04

You expected patients to book a new-patient exam straight from the listing, the way they do through your online scheduler. They cannot.

    What it costs

The patient who would have picked a slot at night on your website leaves a message instead, waits, and is often already booked elsewhere by the time the office calls back.

    What’s happening

Booking leads are not available for health care verticals, so a dental LSA listing produces calls and messages only. Messages arrive through proxy numbers and email aliases, often in the evening, and sit until someone opens the inbox the next morning.

  5. 05

LSA looks cheap per lead, but nobody can say how many new patients it produced.

    What it costs

LSA gets judged on lead count. A channel that looks cheap per call may be expensive per kept first visit, or the reverse, and the budget decision gets made without knowing which.

    What’s happening

LSA leads carry no click ID, the caller is often a parent or spouse while the chart is in someone else's name, and the referral source dropdown in the practice software says 'Google' for Search, LSA and the map listing alike.

The goal

What a fixed LSA account looks like for dentists

  • Every LSA lead has an outcome from the front desk: new patient booked, existing patient, insurance not accepted, not a patient, or missed.
  • Cost per kept new-patient visit from LSA, next to Search, from the practice software, with no treatment detail leaving the office.
  • Services switched off for work you refer out, and the bio stating the plans you accept, so fewer calls end at the insurance question.
  • Listing hours that match the hours someone answers, including lunch and short days.
  • Recall and reschedule traffic steered to the office number, and the share of LSA leads that are patients of record tracked month to month.

Eligibility

What Google checks before dentists can run LSA

  • 1A provider background check for each dentist on the listing, plus a practice background check.
  • 2A state-level practice license and a provider license for each dentist, where local law requires them. Each dentist clears their own check, so add a new associate to the listing once their license paperwork is ready rather than holding up the whole profile.
  • 3General liability insurance for the practice and professional liability or malpractice insurance for each provider, which Google asks of selected businesses.
  • 4National Provider Identifier verification and a business registration check, also for selected businesses. Health care verticals cannot run pre-badge ads, so nothing serves until screening is finished and the Google Verified badge is live.

Job types

LSA job types for dentists: what to switch on, what quietly costs money

Switch on

  • New patient exams and cleanings

The core of most general practices' growth: people who moved, changed jobs or lost their old dentist. Keep it on, and make sure the first available new-patient slot can be offered on the first call.

  • Emergency dental care for toothaches and broken teeth

The same-day caller in pain picks the first practice that can see them today. Keep it on only if you hold emergency slots in the schedule; an emergency caller told to come back next week is a lead you paid for and lost.

  • Fillings, crowns and root canals for new patients

A new patient who already knows something is wrong books faster than a routine exam and usually stays for hygiene. Keep endodontic work on only if a dentist in the practice does it rather than referring it out.

  • Dental implant consultations, if you place implants in house

Fewer calls and much larger treatment plans. Worth having on if the surgery happens in your office, and worth tracking as its own line because implant callers compare fees and financing differently from exam callers.

  • Sedation dentistry, if you hold the sedation permit your state board requires

Anxious patients often search for sedation specifically and are loyal once they find an office that handles it well. Without the permit, every one of these calls is a referral you paid for.

Switch off, or check first

  • Orthodontics and clear aligners, if you refer them to an orthodontist

Every braces or aligner enquiry is charged whether you treat it or pass it on, and there is no lead credit for health care to recover the fee.

  • Wisdom teeth and other oral surgery you send out

Callers asking for extractions under sedation usually need an oral surgeon. The lead is billed at the same price as a new-patient exam and becomes a referral slip.

  • Children's dentistry, if you refer young children to a pediatric dentist

Parents searching for a dentist for a toddler want a yes on the first call. If your answer is a referral, the listing should not be paying to ask the question.

  • Dentures, full-arch and sleep or TMJ appliances you do not offer

Specialized work with long consultations and its own patients. Leaving it on means paying for calls the front desk will turn away politely.

Bad leads

What a bad LSA lead looks like for dentists

  • An existing patient rescheduling a hygiene visit or asking about a bill.

The most common waste in dental LSA and the hardest to undo, because there is no credit for health care and recall visits are usually months apart, far outside the 15 days in which repeat contact is free. Put the office number and a direct reschedule link on every reminder text and email so patients stop searching for you.

  • A Medicaid patient or a member of a plan you are out of network with, who hangs up once they hear the answer.

Valid to Google. Say which plans you accept, and whether you see Medicaid patients, in the business bio, which Google uses to judge relevance, and log the outcome so you can see how much of LSA spend goes to plan questions.

  • A supply rep, a recruiter, a practice broker or a marketing agency calling the number on the listing.

There is no credit to claim, so the only value in logging it is your own report. Mark it as not a patient so it never inflates the count of LSA leads the front desk is judged on.

  • Someone in pain who calls on a weekend and reaches voicemail.

Not a bad lead, a lost one, and voicemails count as charged leads. Either run the listing only in staffed hours or route emergencies to an on-call dentist or answering service that can book the next morning's first slot.

  • A caller who wants an implant fee over the phone and will not come in for an exam.

A valid lead with a low show rate. Record the reason; if price-only callers cluster under one service, that service's description or its place on the listing needs work.

Lead credits

Ranking

What moves your LSA position as one of the dentists in the auction

  1. 01

    Answering during patient hours

Missed calls may hurt your responsiveness. A dental front desk is busiest exactly when people call, during check-in, checkout and insurance questions, so overflow to a second line or a trained call service protects the ranking as well as the patient.

  2. 02

    Reviews and response time

Rating, number of reviews and average response time count toward profile quality. Ask at checkout after a visit that went well, within your state board's rules on testimonials, and answer reviews without confirming that the reviewer is a patient.

  3. 03

    Message leads

Google says enabling message or booking leads gives customers more ways to reach you. Booking leads are not available for dentists, so messages are the only channel besides calls. Turn them on only if someone replies the same business day.

  4. 04

    Accurate hours

The time of a search is part of how Google matches providers. Hours that show you open at lunch or on a closed afternoon produce charged missed calls and a weaker responsiveness record.

  5. 05

    Bid and budget

Google recommends Maximize Leads. Size the budget to the new-patient openings you actually have this month, so the listing is not buying calls for a schedule that is already full.

How we work

Getting LSA leads into Dentrix, Open Dental or Eaglesoft, with Weave or your phone system logging the call

  1. 1

    What to record

A referral source of Google LSA on the new patient record, kept separate from Google Search ad and Google Maps, plus the number the caller used and the date of the first appointment.

  2. 2

    How to match

Match each charged lead to a new patient record by the caller's phone number and the time of the call, within a window you agree in advance. The caller is often a parent or spouse booking for someone else, so keep the calling number on the family record. Do the matching inside the practice software or a file the practice controls; no patient list or treatment detail goes to Google.

  3. 3

    Lead feedback

With no credits to request, the feedback loop is internal. The front desk marks every LSA lead as new patient booked, existing patient, insurance not accepted, not a patient or missed, and that list decides which services, hours and bio wording change next month.

  4. 4

    Monthly check

Monthly: LSA leads, matched new patients and kept first visits, with cost per kept new patient next to the same figure for Search, both from the practice software. Count kept visits rather than booked ones, because a new-patient no-show costs the chair time as well as the lead fee.

No accuracy guarantee here: we report the share of LSA leads for dentists matched to new patients, and the share we could not match.

Channel split

LSA, Search ads and the Business Profile for dentists

For a dental practice, LSA sits above the text ads on 'dentist near me' and hands you a phone call from someone who wants an office close by. Search ads do a different job: pages for implants, emergency care or sedation that list the plans you accept, hours you choose, and new-patient conversions you can send back to Google Ads without any treatment detail. Health ads cannot use remarketing lists or Customer Match, so neither channel can be steered by a patient list; each has to be judged on new patients matched in the practice software. Brand searches are where they overlap and where dental LSA leaks most: a patient of record who searches the practice name and calls through LSA costs a lead fee for a cleaning that was already on the books.

Local Services Ads
New-patient and same-day emergency calls from the neighborhoods around the office, judged on kept first visits.
Search ads
Implants, sedation and emergency care with their own pages, insurance stated up front, and brand terms kept apart from new-patient campaigns.
Business Profile
Reviews, hours and the website and appointment links patients tap from the map. Tag those links and track its calls separately from LSA.
Referral services and directories
Pay-per-call and matching services only where the practice software shows kept new patients at a cost you accept. A transferred call never carries a click.

Google Business Profile

The Business Profile side for dentists

Primary category
Dentist
Secondary
Dental clinic
Suspension risk
Moderate, higher for keyword names and practitioner profiles. Spam listings named 'Best Emergency Dentist Near Me' have trained Google to look hard at dental names, so a profile name that differs from the sign on the door invites trouble. Practitioner profiles are the other trap: Google's guidelines set specific formats for a solo dentist at a branded practice and for dentists at a multi-provider office, and an associate who left often still has a profile at your address carrying your phone number.
Address
Dental practices are storefronts: show the address, keep the hours accurate and the name identical to the signage. Mobile dentistry is the exception and should run as a service-area profile with the address hidden.

Honest fit check

When LSA is not worth it for dentists

  • The next new-patient opening is weeks away. LSA sells nearness and speed, and a caller told to wait books elsewhere while you pay for the call.
  • The front desk cannot answer reliably during business hours. With no credits in health care, every missed call that reaches voicemail is spend you cannot recover.
  • You mainly want full-arch or cosmetic cases. LSA brings the full mix of dental callers, and Search campaigns with their own pages aim at treatment type far more precisely.
  • Most of your callers would be patients of record. A practice whose name is already well known locally may find LSA mostly charges for its own recall traffic.

FAQ

Questions dentists ask about Local Services Ads

Is Local Services Ads worth it for dentists?

It is worth it when cost per kept new-patient visit, measured in your practice software, beats your other channels. Because health care leads cannot be credited, the practices that do well with it are the ones that keep existing patients and plan shoppers off the listing in the first place.

Can dentists get credits for bad LSA leads?

No. Google states that lead credits are not available for health care verticals, so the automatic credits other trades receive do not apply. Prevention is the whole game: services, hours, the business bio and making your office number easy for patients of record to find.

Why can't patients book an appointment from our LSA listing?

Booking leads are not available for health care verticals. Dental listings take calls and, if you enable them, messages. Your online scheduler still works for patients who reach your website, but not from the listing itself.

Can we listen to LSA call recordings?

Not in the US. Google does not provide call recordings for health care verticals located in the United States. If you want call review, use your own phone system's recording with the consent notice your state requires and counsel's approval.

What does Google check before a dental practice can run LSA?

Provider and practice background checks, state practice and provider licenses where local law requires them, and, for selected businesses, insurance, business registration and National Provider Identifier checks. Health care cannot run pre-badge ads, so the listing waits for the Google Verified badge.

How do we track LSA patients in Dentrix or Open Dental without sending patient data to Google?

Record Google LSA as the referral source, keep the calling number on the record, and match the LSA lead list to new patients inside the practice each month. Nothing goes back to Google, so there is no patient data to protect in transit. It is a match, not a click-based import, and we report it as a matched share.
