Your Formstack intake forms get filled, but which ads bring patients who actually book?
Your ads send people to a Formstack referral or appointment-request form, and the ad platforms either show nothing or count every submission, records requests included. Formstack's HIPAA setup limits integrations and restricts webhooks, so the usual shortcuts aren't there. Fixed means a click ID saved on each submission, a conversion that carries no patient answers, and scheduled consults sent back to Google and Meta from your intake records.

Formstack · what breaks
What goes wrong with Formstack HIPAA forms and your ads
- 01
Google Ads shows zero leads from the intake form
When the form opens on a formstack.com link or inside an iframe, your website's tags never see the submit. Smart bidding chases cheap clicks and you can't defend the budget to the practice administrator.
- 02
A records request counts the same as a new referral
Formstack's Google Analytics plugin sends the same form submit event for every entry, so an existing patient updating insurance looks identical to a new oncology or cardiology referral. Bidding learns to buy whatever fills forms fastest, not the patients your schedulers can book.
- 03
You're not sure what the tags are sending
A Pixel or GA tag on a page whose title or URL names a condition can carry that detail to an ad platform, and nobody has checked what each request actually holds. You can't answer your compliance officer, and you can't trust the conversion count either.
- 04
No way to tie a scheduled consult to an ad
Formstack's Healthcare plan limits integrations to an approved list with no ad platform or Zapier on it, so nothing carries 'scheduled' or 'admitted' back to Google or Meta. A hospice or home health agency can't see which campaign fills the census and which one just fills the inbox.
Formstack · the fix
What changes after the fix — Formstack
A conversion with nothing clinical in it
One event per genuine submission with a generic name, a click ID and an event ID. No field answers, no condition in the page path.
Bidding trained on patients you scheduled
Your intake team's scheduled status goes back to Google Ads as an offline conversion matched on the stored click ID, so campaigns are judged on consults rather than form fills.
Spend you can defend per service line
Each Formstack form, say a second-opinion request versus a new-patient referral, reports as its own conversion, so you see which service line every campaign actually feeds.
Formstack · architecture
What Formstack's HIPAA setup actually lets us hook into
Taken from Formstack's help center. Healthcare accounts switch some features off, so each item is confirmed on your plan before we build on it.
- 1
Three embed methods and a hosted link
JavaScript embed (Formstack's recommended method), iframe, lightbox or a formstack.com URL. Only the JavaScript embed puts the form inside your page, where your own container can see it.
- 2
Google Analytics plugin in the Conversion Kit
Sends GA4 events including form start and form submit. It works on the hosted form link, and on your site only through the JavaScript embed. The Conversion Kit is a paid add-on, so we confirm it's on your account and accepted by your compliance team.
- 3
Hidden fields and URL prefill
Hidden fields save with the submission but never show to the patient. URL prefill works on hosted links, though Formstack doesn't support it on embedded forms and warns against putting PII or PHI in URLs, so we use it for click IDs and UTMs only. Its Campaign Tracking add-on fills UTM fields on JavaScript embeds, but not inside an iframe.
- 4
Healthcare restrictions that remove the shortcuts
Healthcare plans can't append submitted data to a redirect URL, limit integrations to an approved list such as Salesforce and Google Sheets, and restrict submit actions and webhooks.
- 5
Forms API for submission records
The API can retrieve submissions, but access is arranged through Formstack sales. Without it, a scheduled export or your Salesforce records feed the offline uploads.
Formstack · diagnose
Where Formstack HIPAA tracking breaks, checked in this order
On a healthcare form the first question is what leaves the page. Only then do we ask whether the right conversions are counted.
- 01
List every request the form page makes
One test submission with the network panel open, noting every call to Google or Meta and what it carries.
- 02
Check page titles and URLs for clinical terms
A path like /cancer-second-opinion rides along with every pageview tag.
- 03
Confirm how each form is published
An iframe or hosted formstack.com link explains missing conversions straight away.
- 04
Submit from a gclid-tagged URL and open the submission
If the hidden click ID field is empty, no offline upload can match it to an ad.
- 05
Compare scheduled patients with reported conversions
Submission by submission over a closed month.

The QA evidence on your live Formstack form
Run on the live form, published the way patients actually reach it, with a test record your team deletes afterward.
Source of truth
Formstack submission records plus your intake team's scheduling status for each submission, by date and service line
- One test submission from a gclid-tagged link, with the click ID visible on the Formstack submission record.
- A network capture showing no field values, names or condition terms in any request to Google or Meta.
- Exactly one conversion per submission in Google Ads and Meta, browser and server events sharing one event ID.
- Records requests and existing-patient forms checked and missing from the new-patient conversion, as intended.
- Scheduled consults uploaded by click ID and matched against Google Ads over a fully closed window.
- Rechecked 30 days later and whenever a form is republished or switched to a different embed.

Formstack · when not to buy
When Formstack practices can skip this
Plenty of practices on Formstack's HIPAA add-on are better off leaving their tracking alone.
- If your Formstack forms only go to existing patients, like intake packets sent after the appointment is booked, there's no ad conversion to track.
- If most new patients arrive by physician referral or phone and paid search is a small line item, call tracking will tell you more than form tracking.
- If your compliance officer rules that no ad platform should receive anything tied to an intake form, not even a click ID, we'll say so rather than sell a build.
- If the only fault is one iframe that should be a JavaScript embed, switching it takes minutes and you can do it yourself.

FAQ
Formstack questions, answered
We wouldn't. Anything a browser tag can read on an intake page, like the URL and page title, becomes something you have to defend, so we send a content-free conversion from a server container instead and your compliance officer approves exactly what leaves.
Not on the Healthcare plan: its approved integration list covers tools like Salesforce and Google Sheets, with no ad platforms and no Zapier. Conversions reach Google through a tag or an offline upload you control.
Usually the form sits on a formstack.com link or inside an iframe, so your site's tags never see the submit. Formstack's own GA plugin can't help on an iframe either: on your site it only works through the JavaScript embed.
That's a call for your compliance officer or counsel. What we control is the data map: only a click ID, event ID and timestamp leave, and you approve it in writing before launch.
Yes, as offline conversions matched on the click ID saved in a hidden field on the Formstack submission, with no clinical detail attached.
Often not. Google's health personalized-advertising policy doesn't support advertiser-curated audiences such as Customer Match for health promotions, so the build focuses on conversion signals for bidding rather than audiences.
